Job description
The primary functions of the Care Management Registered Nurse are to lead and oversee clinical interventions and outreach efforts for patients in order to help promote comprehensive, coordinated quality care. This is accomplished via one-on-one meetings, didactic sessions focusing on evidenced-based best practices, and mentoring. The Care Management Registered Nurse works closely with providers for support and guidance to ensure quality patient care and service in compliance with standards, laws, and regulations as directed by regulatory and accrediting organizations, such as but not limited to OSHA, HRSA, and State and Federal Government guidelines pertaining to FQHCs. The Care Management Registered Nurse maintains nursing operations by initiating, coordinating, and enforcing program, operational, and personnel policies and procedures.
Qualifications
Qualifications
- Graduate from an accredited School of Nursing with a Bachelor of Nursing Degree
- Hold current or eligible for RN license in the Commonwealth of Pennsylvania
- Public health/community health experience preferred
- Experience working with a second culture required
- Experience working at a Federally Qualified Health Center preferred
- Holds current CPR certification
- Maintains current nursing continuing education credits as mandated by the state of PA, including child abuse recognition and training
- Bilingual proficiency in English and Spanish preferred.
