Care Coordinator, CCR Program, STAR-Brookdale – One Brooklyn Health – Brookdale Hospital Medical Center – Brooklyn, NY. The STAR Health Center at Brookdale Hospital Medical Center is a comprehensive ambulatory care program caring for the most vulnerable populations in Brooklyn, including people with HIV and HCV, immigrants, LGBTQ and gender nonconforming populations, and people with mental health and substance use disorders. Duties: Care Coordinators supervise, mentor, and guide patient navigators. They monitor navigation services for each patient on their caseload to ensure that they are conducted according to program standards. They are responsible for leading comprehensive assessments, organizing case conferences, and ensuring service planning. The Care Coordinator is usually the first point of contact for patients enrolling in the program. They are responsible for introducing patients to the program, conducting the initial comprehensive assessment and service plan development, and incorporating a patient’s medical treatment plan into the overall service plan. Care Coordinators also facilitate case conferences with the primary care provider, patient, and other members of the care team. As needed, Care Coordinators may also cover services normally provided by the Patient Navigator. Required Qualifications: BA/BS. Experience working in multicultural, team-oriented setting preferred. Experience working with PLWH, substance using and/or MICA clients and experience working with the LGBTQ population preferred. Strong communication skills both written and verbal, with the ability to interact with all employees, staff, and senior leadership. Computer literacy required, and experience using electronic health record strongly preferred. Bilingual in English/Spanish or English/Haitian Creole preferred. Ability to be decisive based on prior experience, precedent or interpretation of regulations/policy. Excellent organizational, prioritization, time management skills. COVID vaccination is required. Masks must be worn at all times. Preferred Qualifications: Master’s degree. Hours: Full-time, M-F, 9am-5pm, 37.5 hours per week. Salary Range: $60,000.
