Care Coordination Specialist Position Summary

The primary function of the Care Coordination Specialist is to provide psychosocial support to patients and their families receiving treatment in the Richard A. Henson Cancer Institute. The responsibilities include assisting licensed social workers and navigators, providing social support to patients and families, identifying and obtaining available community resources, and participating in the planning and implementation of outreach activities. The Care Coordination Specialist may develop and organize programs to prevent and resolve barriers to care. This position will split their time between the Salisbury and Ocean Pines office and may include Outreach events in Sussex County as well.

Care Coordination Specialist Position Requirements

Education

• Associate’s or Bachelor’s degree in Social Work, Social Sciences, Human Services, or related field

Experience

• Experience assisting social work/navigator in a healthcare setting preferred.
• Oncology experience preferred.

• Knowledge of community resources and social service programs and how they can be utilized on behalf of the patients.
• Strong interpersonal and organizational skills.
• Must possess good interpretive and communication skills, both written and oral.
• Demonstrates sensitivity and empathy in dealing with patients and families under stress.
• Demonstrates ability to prioritize tasks and manage time effectively.
• Demonstrates ability to problem solve and is able to work independently.
• Works under pressure of time and shifting priorities.
• Knowledge of patient’s rights, role of advocate, and use of this role in helping patients and families determine the best plan of action.
• Knowledge of computer skills.

• Contributes to department budget development and efficient use of department resources.

• Frequent contact with patients and families.
• Frequent contact with medical and other hospital staff and departments.
• Frequent contact with community agencies and the public.

Care Coordination Specialist Work Schedule

• Day shift Monday-Friday.
• Occasional evenings and weekends when Outreach Events warrant it

• At TidalHealth, team members working at least 36 hours per pay period based on 12-hour shift schedules or at least 37.5 hours for non 12-hour shift schedules and part-time team members working at least 30 hours or more on weekends only are eligible for benefits.
