JOB SUMMARY

The Behavioral Health Integrated Specialist is the team member that works one to one with individuals interested in receiving an integrated medical and behavioral/substance use disorder model of care. This team member takes the lead on assessing interested patients, designing care coordination plans and coordinating care management for their assigned patients at NHCLV. Our model of care also includes a community-based team that supports each patient and works alongside him/her to reach goals. This role carries a caseload.

The Behavioral Health Integrated Specialist works both in the health center & also in the community- so it's an active role. This role needs someone who likes to work with people, who is resourceful, enjoys motivational interviewing, helping others to set goals, and can be a productive, positive member of a larger team of care providers.

ESSENTIAL FUNCTIONS

1. Create a care management plan based on the ASAM Level of Care Determination for each COE patient
2. Coordinate and facilitate communication between patient, primary care physician, consulting psychiatrist, mental health providers, drug and alcohol counseling partners and family members when appropriate, in accordance with confidentiality requirements for persons undergoing substance use treatment.
3. Based on the information gathered through the ASAM, facilitate treatment linkages for patients to the appropriate level of care, including inpatient detox, residential treatment for mental health and/or substance use disorder, and outpatient services as needed
4. Ensure that all patients in the COE program have completed all required paperwork (including ASAM Level of Care Determination, all appropriate consent forms, etc.) before initiating entry into care management services.
5. Monitor screening of all adult patients for depression through the use of appropriate tools; This may include: the Patient Health Questionnaire (PHQ-9) depression scale. Conduct as needed/assigned GAD, ASAM, LOC, PA state surveys, SDOH, and general needs.
6. Conduct warm handoffs at initial patient encounter initiated by staff that meets the criteria for COE or behavioral health program to determine behavioral health needs.
7. Monitor patients closely (in-person or by phone) for changes in severity of symptoms, co-morbidity issues, co-occurring disorders and medication side effects; educate patients about medications and medication side effects, as needed; encourage treatment adherence; perform administrative tasks related to projects; Monitor outcome measurements such as PHQ-9, PHQ-A, and PA state survey, as appropriate.
