Our candidates will need to possess a Baccalaureate degree in a health care related field and have two years' professional experience with long term care populations. JOB DESCRIPTION
Job Title: ADvantage Case Manager (Non-Nurse)
Supervisor: ADvantage Case Management Supervisor
Job Summary:
The position holder is responsible for the ADvantage case management process, which includes admission/intake, assessment/reassessment, service planning, service plan implementation, monitoring/evaluation, and discharge of members. The roles assumed by the ADvantage case manager within the long-term care service delivery system are service coordinator, ADvantage member advocate, ADvantage member consultant, and gatekeeper.
1. Access – Facilities access to ADvantage case management services for ADvantage members.
2. Intake, Screening, & Referral – Establishes intake, screening, and referral procedures for information requests about community long-term care options.
3. Admission – Admits only those ADvantage members determined eligible, and referred to the Case Management Case Management.
4. Assessment/Reassessment – Reviews the comprehensive assessment, and/or completes reassessment, as needed, to identify ADvantage member needs, goals, abilities, resources, and supports as the basis for service planning.
5. ADvantage Member Orientation/Education – Orients and provides on-going education to ADvantage members as needed to empower them as informed and active ADvantage members.
6. Resource Coordination – Ensures coordination of available community resources and informal support, for which the ADvantage member is eligible and/or has needs.
7. Service Plan Development – Submits a comprehensive, written service plan for each ADvantage member that was developed using the interdisciplinary team process; that establishes service goals to meet identified ADvantage member needs and promote independence, that uses the most effective and cost efficient formal and informal resources, and that clearly defines implementation responsibilities.
8. Supervisor/Management Review – Accepts direction and feedback from the ADvantage Case Manager Supervisor.
9. Service Plan Implementation – Implements the approved service the plan.
10. Monitoring, Evaluation, and Review – Monitors the delivery of services and evaluates member outcomes and makes revisions as indicated.
11. High Risk/Special Needs Members – Uses risk-management mechanisms to address high-risk situations and special needs of ADvantage members.
12. Changes in Service Status – Modifies the service plan based on the status of the Advantage member without disruption of services or threat to the ADvantage member’s health and safety.
13. Emergencies – Reviews and monitors direct service provider emergency procedures to protect the health and safety of the Advantage member.
14. Collaboration - Collaborates with staff Registered Nurse regarding medically complex members.
15. Confidentiality – Ensures confidentiality according to all relevant laws and regulations.
16. Reporting and Recordkeeping - Maintains appropriate and adequate records to document activity in the performance of responsibility as a component of the service delivery system.
17. Miscellaneous – Participates in all other job duties as requested and/or assigned by the Advantage Case Management Supervisor or Director.
18. Training/Education – Participation in all training and continuing education as required.
Job Qualifications
The applicant must possess a baccalaureate degree in a health care related field and have two years’ professional experience with long-term care populations. Requires completion of the ADvantage Case Management training

1. Requires general management skills with an emphasis on communication, collaboration, and interpersonal skills.
2. Requires excellent verbal and writing skills.
3. Requires attendance at BHCPS staff meetings, case conferences, and other meetings and requested/assigned.
4. Requires intermediate computer skills using an office based word processing system.
Other Requirements
1. Access to reliable transportation.
2. Finger dexterity for writing and typing.
3. Ambulatory type work occasional sitting, frequent standing or walking exertion up to ten pounds.
4. Speak fluent English

Job Type: Full-time

Pay: $17.00 - $20.00 per hour
