TITLE: Access and Resource Program Coordinator

HOURS: Full-Time (35 hours per week) - Monday-Friday (some evenings, and occasional weekends

PROGRAM: Hyannis

SALARY: $65,000 Annually

DUTIES: Health Imperatives seeks an Access and Resource Coordinator who will implement new initiative to assist immigrant clients in obtaining essential resources and conduct outreach in immigrant communities to engage individuals and families in services.

The Access and Resource Program Coordinator will:

  • Conduct thorough intake and needs assessment in partnership with each client to identify the client’s immediate and longer-term needs.
  • Create an individualized service plan with each client that includes specific action steps to help clients meet their basic needs, improve their health, pursue goals for education and/or employment, and move towards self-sufficiency.
  • Refer and/or accompany individuals for services within Health Imperatives and/or at other community-based agencies and organizations on Cape Cod.
  • Follow up with clients to ensure their needs have been met; troubleshoot and constructively resolve any challenges or barriers that clients encounter.
  • Working in conjunction with HI staff, engage immigrant individuals and families and familiarize them with the services and resources available to them on Cape Cod.
  • Recruit, hire, and supervise four immigrant peer leaders to facilitate immigrant engagement.
  • Provide information to a wide range of audiences, including immigrants and refugees, social services providers, community-based organizations, using existing materials or creating new ones.
  • Establish or build upon partnerships and referral networks with internal and external collaborators.
  • Coordinate a collaborative effort to improve and expand services for immigrants on Cape Cod.
  • Meet with staff of nonprofit organizations to build organizational knowledge on immigrant access to various systems.
  • Work in conjunction with team to coordinate an immigrant resource fair.
  • Participate in Peer Learning Meetings.

QUALIFICATIONS:

High school diploma/General Education Diploma (GED) and three years of experience conducting case management and/or outreach or a combination of life/work experience. Must be knowledgeable about the strengths and needs of immigrant communities and adept at navigating health care or social service systems on Cape Cod. Demonstrate interest and knowledge in relevant subject areas (e.g. refugee and immigrant access to benefits); Excellent writing and presenting skills; strong organizational and project management skills.

Preferred Qualifications
Prior experience conducting outreach activities and connecting clients to required services or resources. Graduation from an accredited college or university with an associate degree or higher in psychology, social work, counseling, human services, or a closely related field. Excellent communication skills, facilitation skills, and relationship-building skills. Multilingual required; Multicultural preferred.
